After we receive your payment we will send you an email within 2-5 business days with your personalized medical applications forms. These need to be filled out by you and your doctor. The completed documents and tests results then need to be sent to us at:

This payment is for review only. All charges for the doctor , dentist and testing are your responsibility. Please be advised that there is an extra $60.00 charge for redoing temporary ,limited or return to work clearance medical certificates. All Sea Medical is a review service. If the payment is being made by someone other than the crew member please note that during the payment process. By clicking on this link you agree that the doctors and employees of All Sea Medical can access and review all your medical records.
